pp4danBarnet wants to hear your views about the service you are receiving if your young person is going through Transition from Children's to Adult's Services which starts in Year 9?

1. Are you involved with any of the process? Are you informed and given any information before the Annual Review?

2. Are you told who, when and why your young person is being interviewed to discuss their future plans?

3. Are you invited to give your opinions as to whether your young person will understand the process by being given a copy of the questions being asked?

4. Are you given a Flow Chart as to what actions to expect this year and in the future?

5. Are you given any meaningful information apart from a scant leaflet?

6. Are you given good advice about all the possible, suitable opportunities for your young person including more than the local College?

Your feedback is crucial for parent-carers to ensure the correct services are put in place to give our young people a meaningful preparation for their adult life.
